Checks for completed reimbursement applications are sent to the employee's home address the first week of the month. (A check dated 6/30/04 is typically mailed the first week of July.)
Checks for completed prepayment applications are mailed to the dependent's name/address according to the calendar below:

Note: In most cases, applications selected to receive payment are processed without error. However, the information displayed above is not a guarantee of payment. If a check is not processed, you will be notified by mail.
